anything should help a little bit it lets it breath better and give it more horse power but the key is to keep your foot out of it witch is very hard to you will find yourself getting in the throttle just to hear the sound

An exaust system wouldn't hurt gas mileage, and could even help. I'm not sure about a chip, unless the manufacurere says it increases gas mileage, or says it doesn't affect it.

Both could increase milage, but again a lot of gas milage depends on your driving style.

Intake and exhaust to make it breathe better, a tuner (or custom tune) to improve performance and mileage even more.

expanding on the original question: would lifting the truck a couple inches affect the mpg? What about bigger tires, not oversized but a little bit larger than stock??

Hit the nail on the head with that statement.

Any increase in tire size is going to drop your MPG. You can counteract this by the exhaust, CAI, or by re-gearing. It's up to you.
