Installed HID's in High, and Low beams along with the Hi4 mod. Mounted the ballast's behind grille on the core support (for now using foam tape pads till i get screws) when i turn the lights on they all 4 work great, i drive to work and shut it off, after work i start it and turn them on and the passenger side low doesnt work, but the other 3 do. i shut it off, then a little while later turn them on and it works.

Hi, my name is Wes, I work at Custom Truckk Parts in Edmonton and I see this problem frequently. Check your grounds and the HID wire harness fuse holder for continuity, if there is no problem there, try starting the truck with the park brake ON. It sounds weird but it has something to do with the voltage in the dash. When the HIDs fire, the voltage drops and one light will not stay on.
